I will be providing the COVID-19 vaccine to you today but before I do, I have a few questions to ask you.

      

 

Have you had another vaccine in the past 14 days?

      
  • If no, move to the next question.
      
  • If yes, we would need to reschedule.
      

 

Do you have a history of a severe allergic reaction to a prior vaccine, including a COVID vaccine?

      
  • If no, move to next question.
      
  • If yes, would need interpreter to know what was the reaction and to what vaccine the reaction occurred.
      

 

Do you have a history of a severe allergic reaction to polyethylene glycol (PEG) or PEG-containing product such as Miralax or an injectable steroid?

      
  • If no, move to next question.
      
  • If yes, would need interpreter to know what was the reaction.
      

 

Do you have a history of a severe allergic reaction to a polysorbate or polyoxyl 35 castor oil (e.g. paclitaxel) containing injectable or vaccine?

      
  • If no, move to next question.
      
  • If yes, would need interpreter to know what was the reaction.
      

 

Do you have a history of a severe allergic reaction to another allergen (e.g., food, medication, venom, or latex)?

      
  • If no, can proceed with vaccination.
      
  • If yes, would need interpreter to know what was the reaction. A yes answer does not typically require a virtual visit or provider clearance but does require a 30 minute observation time.
      

 

If able to vaccinate, proceed with the next set of instructions:

      
  • Would you like to receive the vaccine in your right or left arm?
      
  • Please roll up your sleeve all the way to your shoulder.
      
  • Relax your arm down by your side.
      
  • Are you ready?
      
  • You will feel a little pinch.
