I will be providing the COVID-19 vaccine to you today but before I do, I have a few questions to ask you.
Have you had another vaccine in the past 14 days?
- If no, move to the next question.
- If yes, we would need to reschedule.
Do you have a history of a severe allergic reaction to a prior vaccine, including a COVID vaccine?
- If no, move to next question.
- If yes, would need interpreter to know what was the reaction and to what vaccine the reaction occurred.
Do you have a history of a severe allergic reaction to polyethylene glycol (PEG) or PEG-containing product such as Miralax or an injectable steroid?
- If no, move to next question.
- If yes, would need interpreter to know what was the reaction.
Do you have a history of a severe allergic reaction to a polysorbate or polyoxyl 35 castor oil (e.g. paclitaxel) containing injectable or vaccine?
- If no, move to next question.
- If yes, would need interpreter to know what was the reaction.
Do you have a history of a severe allergic reaction to another allergen (e.g., food, medication, venom, or latex)?
- If no, can proceed with vaccination.
- If yes, would need interpreter to know what was the reaction. A yes answer does not typically require a virtual visit or provider clearance but does require a 30 minute observation time.
If able to vaccinate, proceed with the next set of instructions:
- Would you like to receive the vaccine in your right or left arm?
- Please roll up your sleeve all the way to your shoulder.
- Relax your arm down by your side.
- Are you ready?
- You will feel a little pinch.
- The vaccine is all done. Please sit here for 15 minutes for observation. If you have any questions or need any help, please let us know.
